Declaration of euthanasia

Every Belgian citizen with a national registry number can make an euthanasia declaration. The template can be found via : Euthanasia declaration | Federal Public Service Foreign Affairs (belgium.be).

The declaration can be registered in a Belgian town hall, but not by our consulates abroad.

The declaration can be used in Belgium even if it was not registered. A euthanasia declaration cannot always be used abroad.
